Sometimes, putting in the effort yields the exact opposite of what you expect. When discussing diet and nutrition, you might find that eating healthy or strictly controlling calories leads to paradoxical results.
To express a strong contrast between an action and its unexpected outcome, Thai uses the ทั้งๆ ที่... แต่กลับ... frame.
Setting the Expectation: ทั้งๆ ที่
To say "even though" or "despite the fact that," place ทั้งๆ ที่ at the beginning of the first clause. This establishes the context or the action you took.
The Unexpected Result: แต่กลับ
To introduce the contradictory outcome, use แต่กลับ in the second clause.

This compound pairs แต่ (but) with กลับ (instead / on the contrary). By placing แต่กลับ before the verb in the second clause, you emphasize that the result was the exact opposite of the goal.
| Even though I eat clean food, I still gain weight instead. | ทั้งๆ ที่ กิน อาหาร คลีน แต่กลับ ยัง อ้วน ขึ้น ค่ะ |
| Even though he controls his diet, he has bad nutrition instead. | ทั้งๆ ที่ เขา คุม อาหาร แต่กลับ มี โภชนาการ แย่ ครับ |
Notice how แต่กลับ immediately precedes the verb or adjective highlighting the irony:

Logical Consequences
Not everything is a paradox! To express standard, expected outcomes—like what happens when a diet is too extreme—use the ถ้า... (if) frame. You will frequently see the result clause marked with อาจจะ (might) or จะ (will) before the verb:
| If you diet this much, your metabolism might break. | ถ้า ไดเอท ขนาดนี้ ระบบ เผาผลาญ อาจจะ พัง ครับ |
