Why is there an 's' added to gek to make it geks?

You are exactly right that wat is the question word "what". However, in everyday spoken Dutch, wat is very commonly used as a shorter, casual version of iets (something). You could easily change this to Heeft zij iets geks gedaan? and it would mean the exact same thing.
Why is gedaan at the very end of the sentence?
This is a refresher on the present perfect tense (used here to mean "did do" or "has done"). In Dutch yes/no questions, the helping verb (heeft) goes first, and the past participle (gedaan) is pushed all the way to the end of the sentence. The object (wat geks) gets sandwiched between them.
What happens if I want to say she did "nothing crazy"?
You would use the word niets (nothing). Because niets is also one of those indefinite words, the adjective still gets the -s. The sentence would simply change to: Zij heeft niets geks gedaan.
