Why is the phrase vill ha (want to have) split up by the subject?

Swedish questions must have the main verb in the second position. Here, vill is the main verb, so it comes right after vad. The subject (kunden) must follow immediately after that, pushing the second verb (ha) further back.

Why isn't there an att (to) before ha?

Because vill is a modal verb. Just like in English where we say "I can go" instead of "I can to go", Swedish modal verbs take the infinitive form of the next verb without using att.
