By the day: din zi în zi
Din zi în zi (by the day, from one day to the next) describes a change that keeps going. With mai + adjective it means more … every day: se face din zi în zi mai cald (it's getting warmer by the day). It works like din ce în ce mai (more and more), but it names the step. Swap the noun for other steps: din oră în oră (by the hour, every hour), din an în an (year by year). Keep it apart from zi de zi (day after day), which describes a routine, not a change.

Every eight hours: din opt în opt ore
Put a number on both sides and the phrase measures a fixed gap, the wording of every prescription: din opt în opt ore (every eight hours). The noun comes once, at the end, in the plural.

Every other day: tot la două zile
Tot la + number + noun is the everyday way to say every N …: tot la două zile (every other day), tot la cinci minute (every five minutes). La fiecare + number says the same in a neutral tone, and o dată la + number means once every N.

Pe or la?
Pe + one unit gives a rate: o dată pe săptămână (once a week), de două ori pe zi (twice a day). La + a number of units gives the gap between two times, and the unit goes plural:
