Why is it I fantasmi (The ghosts) instead of just Fantasmi?

Since fantasmi is masculine plural, is the singular form fantasmo?
Actually, the singular is il fantasma. It is one of several masculine words ending in -a (often of Greek origin, like il problema or il programma). Even though they end in -a in the singular, they change to -i in the plural just like regular masculine nouns.
Why use nella realtà (in the reality) instead of just in realtà?
Both exist, but they mean slightly different things. Nella realtà (combining in + la) literally means "in the real world." The phrase in realtà without the article is a very common fixed expression that translates closer to "actually" or "as a matter of fact."
If I wanted to say "realities" in the plural, how would realtà change?
It wouldn't change at all! In Italian, nouns that end in an accented vowel (like realtà, città, or università) are invariable. The only thing that changes in the plural is the article: la realtà becomes le realtà.
