Why use j'aimerais instead of je veux here?
Just like in English, it is much more polite to say "I would like" (j'aimerais) than "I want" (je veux) when asking for something or stating a wish. J'aimerais uses the conditional mood, which softens your request so you don't sound demanding.
How exactly is j'aimerais formed?
To build the conditional for regular verbs like aimer, you take the whole dictionary form (aimer) and add the ending -ais for the je form. If you remember the imparfait (past continuous) tense, you will recognize that -ais is the exact same ending!
Why isn't the verb remplir conjugated?
Whenever you have two verbs back-to-back in French, the first one is conjugated (here, aimerais) and the second one stays in its raw dictionary form, the infinitive (remplir). This works just like English: "I would like to fill".
Why is it ce formulaire and not cet or cette?
Formulaire is a masculine noun that starts with a consonant, so we use ce for "this". You only use cette for feminine nouns, and cet for masculine nouns that start with a vowel or a silent h (like cet ami).
