It literally means "in a high voice." This is the standard Spanish expression used whenever you want to talk about reading, speaking, or laughing "out loud" or "aloud."

Why is the adjective alta instead of alto?

Alta is feminine because it has to agree with voz, which is a feminine noun (la voz). Even though it doesn't end in -a, Spanish words ending in -z can be feminine (like la luz and la paz) or masculine (like el lápiz). You just have to memorize their gender!
