Why is it all' instead of al or alla?
all' is formed by combining the preposition a (at) with the definite article l' (the). You use l' here because the noun ingresso (entrance) begins with a vowel. If it started with a regular consonant, you would use al.
How do I know if this is asking one person or a group of people?
The verb siete is the "you plural" form of essere (to be), so we know it is addressing a group of people. If you wanted to ask just one person, you would use sei, making the sentence Sei all'ingresso?.
Why is the word for "you" missing?
In Italian, subject pronouns like voi (you plural) are usually dropped because the verb form (siete) already tells us who is doing the action. Saying Voi siete all'ingresso? is perfectly fine, but leaving the pronoun out is much more natural.
How is this a question if the words aren't rearranged?
Unlike English, which flips the word order (changing "You are" to "Are you?"), Italian keeps the exact same word order for both statements and questions. You simply add a question mark when writing, and raise your voice at the end of the sentence when speaking.
