The perlu family expresses need from several grammatical angles. Bare perlu presents something as necessary, memerlukan presents an experiencer or process as requiring something, diperlukan presents the requirement from the needed thing's perspective, and keperluan packages a need or purpose as a noun. English often uses need for all four, so choosing the Indonesian form requires noticing the sentence's structure, not merely translating one word.
One family, no person or tense endings
None of these forms changes for person, number, or time. Indonesian places time expressions and aspect words around the same form.

Bare perlu is already a complete predicate, not an infinitive that requires another marker before it can be used in a clause.

We require two witnesses before this document is signed.
Data tambahan masih diperlukan untuk memastikan penyebabnya.

The contrast is not tense. Kemarin, sekarang, and besok can all accompany the same form.
Kemarin saya perlu istirahat, tetapi hari ini sudah jauh lebih baik.
Yesterday I needed to rest, but today I am much better.
Bulan depan perusahaan akan memerlukan ruang kerja yang lebih besar.
Next month the company will require a larger workspace.
Bare perlu: something is necessary
Before another predicate, perlu works like an adjective-like modal: perlu diperiksa, perlu pulang, perlu lebih hati-hati. It does not identify tense and it does not agree with its subject. The following word can be a verb, adjective, or passive predicate.
Kamu perlu tidur lebih awal malam ini.
You need to go to sleep earlier tonight.
Harga di situs itu perlu diperbarui setiap pagi.
The prices on that site need to be updated every morning.
Kita perlu lebih sabar menghadapi perubahan ini.
We need to be more patient in dealing with this change.
With a noun, perlu can be a complete predicate: Saya perlu bantuan. This resembles an adjective more than an English transitive verb: Indonesian does not insert an equivalent of English to have or inflect perlu.
Selama masa pemulihan, anak itu perlu perhatian dan jadwal yang teratur.
During recovery, that child needs attention and a regular schedule.
Kalau perlu penjelasan tambahan, hubungi meja informasi.

The sequence perlu untuk + verb occurs, especially in bureaucratic or speech-influenced prose. When the same subject performs the action, however, untuk adds no necessary relation and usually makes the clause heavier; polished and everyday Indonesian normally prefer plain perlu + verb.
Dalam keadaan darurat, petugas mungkin perlu untuk menutup seluruh jalur.
In an emergency, officers may need to close the entire route. (heavy/bureaucratic)
Tidak perlu: absence of necessity
Tidak perlu means do not need to or there is no need to. It removes necessity; it does not normally forbid the action. A prohibition uses jangan or tidak boleh.
Anda tidak perlu mencetak tiket karena kode QR-nya sudah cukup.
You do not need to print the ticket because the QR code is sufficient.
Tidak perlu terburu-buru; keretanya baru berangkat setengah jam lagi.
There is no need to rush; the train does not leave for another half hour.
Compare a lack of necessity with an actual ban:
Tamu tidak perlu melepas sepatu di ruang depan.
Guests do not have to remove their shoes in the front room.
Tamu tidak boleh memasuki ruang penyimpanan.
Guests must not enter the storage room.

Memerlukan: make the requirement transitive
Memerlukan is an active transitive verb. Its subject is the person, project, process, or situation that requires the following object. It is common in neutral and (formal) prose, especially with measured resources, conditions, evidence, skill, and time.
Renovasi gedung tua itu memerlukan biaya hampir dua miliar rupiah.
Renovating that old building requires nearly two billion rupiah.
Pasien ini memerlukan pengawasan ketat selama dua puluh empat jam.
This patient requires close supervision for twenty-four hours.
Mempelajari aksen baru memerlukan latihan yang konsisten.
Learning a new accent requires consistent practice.
With a simple action, perlu + verb is usually the direct choice. Memerlukan instead expects a noun phrase or nominalized event. This is a valency difference, not a difference in politeness.
Saya perlu berbicara dengan kepala bagian sebelum memutuskan.
I need to speak with the department head before deciding.
Keputusan sebesar itu memerlukan pembicaraan dengan seluruh tim.
A decision of that scale requires discussion with the whole team.
The meN- form is spelled memerlukan, not menperlukan: the prefix appears as mem- before a base beginning with p, and the base-initial p is replaced. The general pattern belongs to meN- formation; for this high-frequency verb, learn the whole word as a unit.
Diperlukan and object voice
The passive diperlukan makes the needed entity the subject or topic. It is especially useful when who needs it is unknown, general, or less important than the requirement itself.
Fotokopi paspor diperlukan saat mengambil kartu izin tinggal.
A photocopy of the passport is required when collecting the residence permit.
Berapa banyak kursi yang diperlukan untuk acara besok?
How many chairs are needed for tomorrow's event?
An agent can follow with oleh, particularly in (formal) prose, but Indonesian often omits it when recoverable.
Keahlian itu sangat diperlukan oleh tim tanggap darurat.
That expertise is badly needed by the emergency response team.
With a definite first- or second-person agent, object voice places that agent directly before the bare verb perlukan. The patient remains prominent.
Inilah informasi yang kami perlukan sebelum menyusun anggaran.
This is the information we need before drawing up the budget.
Apakah semua peralatan yang Anda perlukan sudah tersedia?
Is all the equipment you need already available?
Do not insert di- into object voice: yang kami perlukan, not yang kami diperlukan. These options are part of the wider Indonesian voice system.
Keperluan: need, purpose, or business
Keperluan is a noun derived with ke-...-an, but its meanings have become partly lexicalized. It can mean a concrete need, a purpose for which something is used, personal business, or supplies associated with an activity. Context selects the reading.
Dana ini hanya boleh dipakai untuk keperluan penelitian.
These funds may only be used for research purposes.
Dia keluar sebentar karena ada keperluan keluarga.
She stepped out briefly because she had some family business.
Toko itu menjual alat tulis dan berbagai keperluan kantor.
That shop sells stationery and various office supplies.
Keperluan sehari-hari makin mahal sejak awal tahun.
Everyday necessities have become more expensive since the start of the year.
In the fixed question Ada keperluan apa?, the noun asks someone's purpose or business. Depending on tone and setting, it can be neutral or somewhat guarded.
Permisi, ada keperluan apa dengan manajer kami?
Excuse me, what business do you have with our manager?
The adverb seperlunya means as necessary, as needed, or only the necessary amount. It is productive enough to recognize, though its interpretation depends on what is being limited.
Tambahkan garam seperlunya setelah kuah mendidih.
Add salt as needed after the broth comes to a boil.
Bawalah barang seperlunya agar tas tidak terlalu berat.
Bring only what you need so the bag is not too heavy.

Perlu never takes person endings. The prefix in active transitive memerlukan belongs to the established Indonesian verb; it is not agreement.
Using memerlukan directly before an action

Incorrect β memerlukan normally selects a noun phrase, not this bare action complement.
β Saya perlu menelepon teknisi malam ini.
I need to call the technician tonight.
Use perlu + predicate for the subject's necessary action.
Treating tidak perlu as prohibition
β Di area ini, pengunjung tidak perlu merokok.
Incorrect for a smoking ban β this says visitors do not need to smoke.

Visitors may not smoke in this area.
Separating the passive prefix
β Surat rekomendasi di perlukan untuk tahap akhir.
Incorrect β passive di- is attached to the verb.

A recommendation letter is needed for the final stage.
By contrast, locative di is separate: di kantor, di rumah.
Confusing keperluan with the predicate perlu
