Why is กลับ placed before the verb รู้สึก here instead of at the end of the sentence?

You could say ระหว่างที่ฉันแช่น้ำร้อน... but leaving the subject for the main clause flows perfectly and is very common.

It is much better to use ระหว่างที่. You generally use ระหว่าง alone before nouns to mean 'between' or 'during' a specific thing. When you are about to introduce a verb or an entire action clause like แช่ (to soak), adding ที่ makes it a conjunction that properly means 'while doing [action].'
Why don't we need a word for 'I' or 'was' after เหมือน? The English translation says 'like I was in hell.'
Thai is highly context-dependent and avoids repeating pronouns if they are already understood from the main clause. Additionally, Thai doesn't conjugate for past tense, and อยู่ functions perfectly as the active verb 'to be (at a location)' here. So เหมือนอยู่ในนรก directly means 'like being in hell,' which is grammatically complete in Thai without needing extra pronouns or tense markers.
