Indonesian hati names the liver in anatomy, but it also names an inner domain of feeling, character, conscience, intention, and attention. English divides that network differently: its metaphorical heart often corresponds to hati, while the physical heart is normally jantung. This lexical map is culturally specific, but it should not be turned into a stereotype about what Indonesians “believe” physically. Languages simply conventionalize body metaphors differently. English itself uses heart for both a pump and an emotional center without making every use literal.
Hati-hati: a caution word with a hyphen
Hati-hati is a reduplicated caution expression. It can stand alone as a warning, precede an action, or occur after dengan as a manner expression. EYD V spelling requires the hyphen; the hyphen guide covers the wider spelling system.

The conventional farewell hati-hati di jalan conveys care for someone's safety, not suspicion.

Berhati-hati and kehati-hatian
Berhati-hati is a verb meaning “act/be careful.” The prefix attaches to the first element while the reduplicated base keeps its hyphen. It is useful in complete predicates and careful instructions.
Pengguna perlu berhati-hati ketika membagikan data pribadi.
Users need to be careful when sharing personal data. (formal/neutral)
Mohon berhati-hati saat menuruni tangga darurat.
Please exercise caution when descending the emergency stairs. (formal/polite)
Kami berhati-hati agar komentar tersebut tidak terdengar menuduh.
We were careful so that the comment would not sound accusatory.
Kehati-hatian is the abstract noun “caution, carefulness.” It is common in reports, risk management, and fixed principles (formal).
Keputusan itu harus diambil dengan kehati-hatian karena menyangkut keselamatan warga.
That decision must be made cautiously because it concerns public safety. (formal)
Prinsip kehati-hatian diterapkan sebelum pinjaman baru disetujui.
The precautionary principle is applied before a new loan is approved. (formal)
The forms are related but not interchangeable: hati-hati! warns; berhati-hati predicates careful behavior; kehati-hatian names the quality or principle.
Perhatian: attention, care, and polite acknowledgment
Perhatian belongs to the broader hati word family, but its contemporary meanings must be learned directly. It can mean focused attention, concern for someone's welfare, or considerate treatment.
Mohon perhatian, kereta tujuan Surabaya akan tiba di jalur dua.
Your attention, please: the train to Surabaya will arrive on platform two. (formal public announcement)
Anak itu membutuhkan perhatian lebih selama masa pemulihan.
That child needs extra care during recovery.

The verb memperhatikan means pay attention to, observe, or take heed of. Its object follows directly.
Perhatikan tanda di pintu sebelum memasuki ruangan.
Pay attention to the sign on the door before entering the room.
Saya memperhatikan bahwa suaranya berubah ketika topik itu dibahas.
I noticed that his/her voice changed when that topic was discussed.
English pay attention must not be translated word by word with membayar. Indonesian uses memperhatikan or memberi perhatian kepada.
Sakit hati: emotional pain, offense, or resentment
Sakit hati normally refers to emotional hurt, offense, or resentment. It does not ordinarily diagnose liver disease or heart disease.
Raka masih sakit hati karena idenya ditertawakan di depan semua orang.
Raka is still hurt because his idea was laughed at in front of everyone.
Saya tidak berniat membuatmu sakit hati dengan komentar tadi.
I did not intend to hurt your feelings with that earlier comment.
Dia sakit hati kepada temannya yang membocorkan rahasia itu.
He/She feels hurt and resentful toward the friend who revealed that secret.
The causative verb menyakiti hati explicitly means hurt someone's feelings. Patah hati is heartbreak, especially after romantic loss or deep disappointment.
Ucapan yang meremehkan itu menyakiti hati banyak anggota kelompok.
That belittling remark hurt many group members' feelings.
Setelah patah hati, Nisa memilih berhenti membaca pesan lama mereka.
After having her heart broken, Nisa chose to stop reading their old messages.
For physical conditions, use the medical term: penyakit jantung “heart disease,” nyeri dada “chest pain,” or penyakit hati “liver disease,” as appropriate.
Ayahnya menjalani pemeriksaan rutin karena memiliki penyakit jantung.
His/Her father undergoes regular checkups because he has heart disease.
Kecil hati and besar hati: size becomes morale
Kecil hati and berkecil hati describe losing confidence, becoming discouraged, or feeling intimidated. English “small-hearted” is not the normal translation.
Jangan berkecil hati hanya karena usulan pertamamu belum diterima.
Do not lose heart just because your first proposal was not accepted.
Komentar tajam itu membuat peserta baru merasa kecil hati.
That harsh comment made the new participant feel discouraged.
Ia sempat kecil hati, tetapi dukungan temannya membuatnya mencoba lagi.
He/She briefly lost confidence, but a friend's support made him/her try again.
Besar hati is genuinely polysemous: established uses include feeling proud, pleased, or elated, while some contexts can suggest excessive pride. Modern learners will often meet berbesar hati “feel pleased/proud; accept with an open or generous spirit” and membesarkan hati “encourage, cheer.” Context is essential; there is no single English shortcut.
Kami berbesar hati atas kepercayaan yang diberikan oleh warga.
We are deeply pleased and honored by the trust the residents have placed in us. (formal)

Many adjective–hati combinations describe character or willingness rather than a temporary emotion.
Tetangga baru kami baik hati dan selalu menawarkan bantuan.
Our new neighbor is kind-hearted and always offers help.
Meskipun terkenal, penulis itu tetap rendah hati ketika dipuji.
Although famous, the writer remained humble when praised.
Dengan berat hati, kami harus membatalkan kunjungan karena cuaca buruk.
With a heavy heart, we have to cancel the visit because of bad weather.

The network is productive in courteous instructions, thanks, and softened personal interaction.

The last formula is more naturally translated as “we regret” than word-for-word “with a heavy liver/heart.” Phrase-level paraphrase is essential.

1. Translating hati consistently as heart
