The circumfix ke-...-an places one part before a base and another after it, but the two parts form a single derivational construction. In real Malaysian Rumi, the result is one solid word: ke- + baik + -an → kebaikan, not ke baik an, ke-baik-an, or the literal string ke...baik...an.

The structure is:
ke- + BASE + -an → one written word
Both edges belong to the same construction. It is misleading to analyze kebaikan as merely a word beginning with ke- or merely a base followed by -an, because the resulting word belongs to a circumfixed lexical family.

Attachment is stable, not a chain of arbitrary sound changes
The productive template normally preserves the written base. Baik remains visible inside kebaikan; dingin remains inside kedinginan; hilang remains inside kehilangan. Do not apply the root-initial deletion associated with some meN- forms. The circumfix does not turn sihat into ihat or bersih into berih.

Vowel sequences may remain visible when the prefix meets a vowel-initial base. Adil → keadilan contains ke + adil, and ada → keadaan contains the sequence e-a plus the base-final a before -an. Standard spelling, not a learner's attempt to simplify pronunciation, determines the written form.
Keadaan jalan semakin baik selepas hujan berhenti.
The road conditions are improving after the rain stopped. (neutral)
There is no productive allomorph ka-...-an or k-...-an in standard Malaysian spelling. Surface pronunciation may reduce an unstressed vowel in rapid speech (informal), but the written circumfix remains ke-...-an.
The Malaysian form is kesihatan
The Malaysian base is sihat, so the standard derivative is kesihatan: ke- + sihat + -an. Indonesian commonly uses sehat → kesehatan, a legitimate form in that neighboring standard but not the neutral Malaysian form taught here.
Kementerian Kesihatan mengeluarkan kenyataan itu petang tadi.
The Ministry of Health issued the statement this afternoon. (formal Malaysian usage)
This difference is lexical and regional, not an optional vowel alternation inside one Malaysian paradigm. Malaysian learners and writers should not import kesehatan merely because it is frequent in Indonesian media. Conversely, recognizing kesehatan as (regional: Indonesia) is useful when reading across the Malay–Indonesian continuum.
Base categories and lexical restrictions
The construction combines with several types of base. Adjectives frequently yield abstract nouns: baik → kebaikan, bersih → kebersihan, adil → keadilan, besar → kebesaran. Stative or eventive bases yield nouns or words describing an affected state: hilang → kehilangan, dingin → kedinginan, datang → kedatangan. Noun bases also occur in established families such as raja → kerajaan.
Kedatangan tetamu itu disambut dengan persembahan muzik tradisional.
The guests' arrival was welcomed with a traditional musical performance. (formal)
Kerajaan mengumumkan bantuan tambahan untuk mangsa banjir.
The government announced additional aid for flood victims. (formal)
This breadth does not make the construction freely attachable to every adjective, verb, or noun. Ke-...-an formations are lexical: native convention decides which combinations exist and which meanings they have. Kerajaan is not simply the predictable abstract state of being a king; it is the established word for a government, kingdom, or realm in relevant contexts.
Likewise, some formations have more than one grammatical behavior. Kehilangan can be a noun in kehilangan pekerjaan “loss of employment” or part of a predicate in dia kehilangan pekerjaan “he lost his job.” Formation alone cannot decide the syntactic category or meaning in a sentence. Those questions belong to the meaning and usage pages.
Do not confuse the circumfix with independent ke
Independent ke is a preposition meaning “to” and is written separately: ke sekolah, ke Kuala Lumpur, ke dalam bilik. Circumfixal ke- is bound: kebaikan, kesihatan, kehilangan.
Dia pergi ke klinik untuk menjalani pemeriksaan kesihatan.
He went to the clinic for a health examination. (neutral)
In that sentence, ke klinik contains the separate preposition, whereas kesihatan contains bound ke-...-an. The spelling distinction reflects a grammatical distinction; it is not merely typography.
The ending -an by itself also forms many nouns, as in makanan from makan. A word containing ke...an should be assigned to the circumfix only when an attested base and a coherent lexical family support that analysis. Do not strip the first two and last two letters from every matching word.
How dictionaries expose the base
Malaysian dictionaries commonly group derivatives with their base families or provide cross-references that reveal them. Search kebaikan and baik, kesihatan and sihat, kedinginan and dingin, kehilangan and hilang. This confirms the national-standard spelling and shows whether the derivative is a noun, a predicate, or both.
Reverse analysis must be evidence-based. Kemajuan belongs with maju, but not every word beginning with ke- ends in derivational -an; kemudian, for instance, cannot be safely segmented just by its outer letters. Dictionaries prevent learners from inventing a base or assigning the wrong construction.
Formal, informal, academic, and literary uses all retain solid spelling. Register affects which derivative or synonym a writer chooses, not whether the circumfix is spaced. Keadilan sosial is common in formal and academic prose (formal/academic); rasa tak adil may be the everyday conversational paraphrase (informal).
