Clause-initial apakah explicitly marks the following proposition as a yes-no question. Apakah Anda setuju? asks whether the complete proposition Anda setuju is true. The marker is especially useful in forms, interviews, surveys, presentations, and careful writing; spontaneous conversation often uses unchanged word order and intonation instead.

Apakah does not trigger inversion. The subject remains before the predicate, just as it does in the corresponding statement.

Why it often sounds more formal
An intonation question relies on speech melody and shared context. Apakah makes questionhood visible at the beginning, before the reader reaches the question mark. That explicit packaging is valuable in institutional settings and tends to raise formality.

The marker itself does not make the content polite. Titles, pronouns, and wording still determine respect; Apakah kamu ...? is grammatically possible but may combine an explicit marker with an informal relationship.
Optional in direct yes-no questions
For an ordinary direct question, apakah and intonation can ask about the same proposition. The versions differ mainly in explicitness and register.

The shorter versions are normal in everyday conversation. Do not interpret omitted apakah as careless grammar; Indonesian can mark interrogation entirely through prosody. See yes-no questions by intonation.
Embedded apakah: whether
Inside a larger statement, apakah can introduce an embedded yes-no question and correspond to English whether. The whole sentence is not a direct question unless its main clause is interrogative.
Saya belum tahu apakah rapatnya jadi diadakan.
I don't yet know whether the meeting is going ahead.
Tolong periksa apakah pintu belakang sudah terkunci.
Please check whether the back door is locked.
Kami sedang memeriksa apakah semua data sudah tersimpan.
We are checking whether all the data have been saved.
Belum jelas apakah jalan itu aman dilalui malam hari.
It is not yet clear whether that road is safe to travel at night.
The embedded clause keeps ordinary order: rapatnya jadi diadakan, not an English-style inversion. The first, third, and fourth examples are declarative sentences and therefore end in periods.
An entire question can contain an embedded apakah clause:

Here the main question is Bisakah Anda memastikan ...?; apakah berkas ini sudah lengkap is the proposition being checked.
Negative propositions after apakah
Apakah can package a clause containing tidak or belum. The negative remains separate and retains its meaning.
Apakah Anda tidak menerima bukti pembayaran?
Did you not receive the proof of payment? (formal)
Apakah pesanan itu belum dikirim?
Has that order not been sent yet?
Apakah ruangan ini tidak terlalu kecil untuk dua puluh orang?
Isn't this room too small for twenty people?

Register mismatch with casual particles
Everyday speech has sentence-final particles that express insistence, shared knowledge, softening, or stance. Combining formal clause-initial apakah with a strongly casual particle can sound stylistically mismatched, even if listeners understand it.
Apakah kamu sudah makan?
Have you eaten?
Kamu sudah makan siang?
Have you had lunch? (informal)
Choose a consistent frame for the situation. This page does not cover the particles' full pragmatics; the point is that apakah is not a compulsory shell around every spontaneous question.
apakah versus apa and -kah
Standard apakah is written solid: it historically contains attached -kah, but contemporary learners should treat the whole marker as one word. Do not confuse it with question word apa βwhat.β

The first asks for yes or no about a full proposition. The second leaves the object unknown. Attached -kah can also focus other constituents, as explained on the -kah page.
Natural answers
Simple apakah questions accept ya and tidak, but repeating the predicate is often clearer.

The answer follows the predicate's semantics; belum is more informative than bare tidak when the intended answer is βnot yet.β
