Form VIII normally places ت after the first root radical, but weak and hamzated roots can obscure that architecture. Initial-wāw و-ص-ل yields اِتَّصَلَ / يَتَّصِلُ / اِتِّصَال “contact; be connected,” with the root wāw assimilated to ت. Hollow خ-ي-ر yields اِخْتَارَ / يَخْتَارُ / اِخْتِيَار “choose,” with a long vowel in finite stems but a short jussive and command. Doubly weak و-ق-ي yields اِتَّقَى / يَتَّقِي / اِتِّقَاء “guard against; be mindful of,” combining initial assimilation with final-weak mood behavior.
These are distinct, attested outcomes. Do not infer one from another, and do not assume that every initial و, middle و/ي, or final weak root has a current Form-VIII verb. Initial-wāw assimilation: وصل becomes اتصل
The Form-VIII ت normally follows the first radical. In the established verb from root و-ص-ل, first-radical و assimilates completely to that ت, producing doubled تّ:

The initial alif has hamzat al-waṣl. At the start of an utterance, اتصل is ittaṣala; after a vowel, the supporting vowel disappears, as in ثم اتصل thumma ttaṣala. The doubled تّ remains.
اتصلت الطبيبة بالمختبر فور ظهور النتائج.
ittaṣalati ṭ-ṭabībatu bil-mukhtabari fawra ẓuhūri n-natāʾij.
The doctor contacted the laboratory as soon as the results appeared.

In the common “contact” sense, اتصل بـ selects بـ before the person or institution contacted. English uses a direct object, so اتصل المدير المورد is a common transfer error.
اتخذ: a lexical hamzated outcome, not a weak-wāw model
اِتَّخَذَ / يَتَّخِذُ / اِتِّخَاذ, root ء-خ-ذ, means “take; adopt.” It is traditionally analyzed as Form VIII of أخذ, but its root hamza is absent and the ت is doubled. This resembles اتصل on the surface, yet the histories and roots differ. Treat اتخذ as a verified lexical paradigm, not as evidence that every hamzated root deletes hamza in Form VIII.
اتخذت اللجنة قرارين مؤقتين لحماية الموظفين.
ittakhadhati l-lajnatu qarārayni muʾaqqatayni liḥimāyati l-muwaẓẓafīn.
The committee adopted two temporary decisions to protect the employees. (Formal.)
لن تتخذ المديرة إجراء جديدا قبل استشارة الفريق القانوني.
lan tattakhidha l-mudīratu ʾijrāʾan jadīdan qabla stishārati l-farīqi l-qānūniyy.
The director will not take a new measure before consulting the legal team. (Subjunctive after لن.)

The two experts did not take a final position on the proposal. (Neutral VSO; jussive.)
Unlike a final-weak or hollow verb, يتخذ keeps its stem consonants in the jussive; only the mood ending changes. Similar spelling does not imply identical weak behavior.
Hollow Form VIII: اختار has long and short stems
Root خ-ي-ر produces اِخْتَارَ / يَخْتَارُ / اِخْتِيَار “choose.” The maṣdar spelling اختيار makes ي visible, while the perfect and imperfect finite stems contain long ā. Before consonantal perfect suffixes and in the suffixless singular jussive, the vowel shortens: اِخْتَرْتُ “I chose,” لَمْ يَخْتَرْ “he did not choose.” The masculine-singular command is the same short stem without the person prefix: اِخْتَرْ “choose!”

Feminine command اختاري and dual اختارا retain the long vowel before their vowel-led endings; masculine singular اختر and feminine plural اخترن use the short stem.
اتقى: assimilation at the beginning, weakness at the end
اِتَّقَى / يَتَّقِي / اِتِّقَاء, root و-ق-ي, is doubly weak. The initial wāw assimilates to ت, as in اتصل, while the final yāʾ behaves like a final-weak radical. In contemporary formal MSA, the verb means “guard against; avoid,” often with a direct object, and “be mindful/fear” in formal or religious contexts.
Indicative يتقي retains final yāʾ, subjunctive لن يتقيَ retains it with overt fatḥa, and jussive لم يتقِ deletes it. The masculine-singular imperative is اِتَّقِ; feminine singular اِتَّقِي and masculine plural اِتَّقُوا show their own addressee endings.
يتقي العاملان استنشاق الغبار بارتداء قناعين واقيين.
yattaqī l-ʿāmilāni stinshāqa l-ghubāri birtidāʾi qināʿayni wāqiyayn.
The two workers avoid inhaling dust by wearing two protective masks. (Neutral VSO.)

Passive and participial contrasts require vocalization
Form VIII can form a passive, but weak stems may make active and passive forms visually close in unvocalized text. Hollow اختار has perfect passive اُخْتِيرَ “was chosen” and imperfect passive يُخْتَارُ “is chosen.” Its active imperfect is يَخْتَارُ. The written consonants are identical in the imperfect; prefix vowels distinguish the readings when vocalized.
اختيرت المرشحتان بعد مقابلات استمرت يومين.
ukhtīrati l-murashshaḥatāni baʿda muqābalātin stamarrat yawmayn.
The two women candidates were selected after interviews that lasted two days. (Formal passive; singular verb before a dual subject.)
Do not use an active interpretation merely because English omits an agent. Arabic voice is encoded inside the stem, and the following nominative participant is the passive subject.

Restoring initial wāw in اتصل
❌ اوتصلت الطبيبة بالمختبر.
Incorrect—the established Form-VIII verb from و-ص-ل has complete assimilation: اتصلت.
✅ اتصلت الطبيبة بالمختبر.
ittaṣalati ṭ-ṭabībatu bil-mukhtabar.
The doctor contacted the laboratory.
Omitting the complement preposition with اتصل
❌ اتصل المدير المورد صباحا.
Incorrect for ‘contacted the supplier’—اتصل selects بـ before the contacted party.

Keeping the hollow long vowel after لم
