There is no single Indonesian equivalent of the Western sequence “first name + middle name + inherited last name.” One person may have one official name, another several personal name elements but no family name, another a marga or inherited family name, another a patronymic with bin or binti, and another a Balinese sequence containing title, gender, or birth-order information. The only safe starting point is the person's own full documented name.
This page explains the language and form design needed to preserve that diversity. It does not turn naming traditions into tourist facts or infer ethnicity, religion, gender, or kinship from spelling alone. Naming practices vary within every community; the name-as-you guide develops how a chosen name then functions in conversation.
Nama lengkap is one complete identity string
Nama lengkap means “full name.” It is not a promise that the final word is a surname or that every earlier word is a given or middle name.
Nama lengkap saya Rahayu.
My full name is Rahayu.
Nama lengkap saya Muhammad Rizky Pratama, dan saya biasa dipanggil Rizky.
My full name is Muhammad Rizky Pratama, and I usually go by Rizky.
Silakan tulis nama lengkap persis seperti pada dokumen kependudukan.
Please write your full name exactly as it appears on your population document.
In the first sentence, Rahayu is complete, not a missing first or last name. In the second, Rizky is a chosen call name, but the sentence does not reveal whether Pratama is a family name. Ask rather than parse.
One-word names still exist in valid documents
Single names are an established part of Indonesian naming diversity. Current administration adds an important date-sensitive distinction. Permendagri Nomor 73 Tahun 2022 sets conditions for first-time name recording, including at least two words and no more than 60 letters including spaces. Its transitional provision also says population-document names recorded before the regulation remain valid.
That means two statements can both be true: current first-time registration is governed by a two-word rule, and many people legitimately continue to hold valid one-word names. A form must accommodate the people who actually possess those documents.
Nama pada KTP beliau hanya satu kata: Rahayu.
The name on her identity card consists of only one word: Rahayu.
Petugas menyalin Rahayu ke kolom nama lengkap tanpa menambahkan nama keluarga.
The officer copied Rahayu into the full-name field without adding a family name.
Dokumen lama itu tetap berlaku meskipun nama pemiliknya terdiri atas satu kata.
That older document remains valid even though the holder's name consists of one word.
Do not duplicate a single name into two fields as Rahayu Rahayu or invent a relative's name. If an international system demands “given name” and “surname,” follow the person's passport and the receiving institution's documented procedure.
Several words do not prove a surname
Multiword names can contain several personal choices with no inherited family element. Indonesian conversation normally uses nama lengkap and nama panggilan rather than forcing every part into “first/middle/last.”
Nama lengkapnya Dewi Anggraini Putri, tetapi teman-temannya memanggilnya Dewi.
Her full name is Dewi Anggraini Putri, but her friends call her Dewi.
Apakah Pratama nama keluarga Anda atau bagian lain dari nama lengkap?
Is Pratama your family name or another part of your full name?
Saya tidak memakai nama keluarga; semua unsur itu merupakan nama pribadi saya.

The second question is appropriate in an administrative clarification (formal) because it asks the owner. A database that silently labels the last token surname replaces diversity with a guess.
Marga and inherited family names
Some Indonesian communities use inherited marga or family names. In many Batak naming systems, for example, a marga such as Simanjuntak carries important kinship information. Other communities in Indonesia also use family names, but their inheritance and social meanings are not identical.
Nama lengkap saya Daniel Simanjuntak; Simanjuntak adalah marga saya.
My full name is Daniel Simanjuntak; Simanjuntak is my marga.
Pada formulir itu, Daniel menulis Simanjuntak di kolom marga sesuai identitasnya sendiri.
On that form, Daniel writes Simanjuntak in the marga field according to his own identity.
Jangan menghapus marga ketika menyalin nama lengkap dari dokumen resmi.
Do not delete the marga when copying the full name from an official document.
Marga is often mapped to “surname” for an international form, but the translation does not capture all its kinship functions. Conversely, not every final name element is a marga. The person, not the software, supplies that classification.
The 2022 regulation explicitly allows a marga, famili, or comparable name to be included in population documents and treats it as one unit with the name. This supports preservation, not automatic assignment.
Patronymics with bin and binti
Some Indonesian Muslim names contain Arabic-derived bin “son of” or binti “daughter of” followed by a parent's name. These are patronymic relations, not universal Muslim name requirements and not automatically hereditary surnames.
Nama lengkapnya Farid bin Hamzah.
His full name is Farid bin Hamzah.
Nadia binti Yusuf meminta agar seluruh unsur namanya dicetak lengkap.
Nadia binti Yusuf asked for every element of her name to be printed in full.
Pada daftar peserta, nama Farid bin Hamzah tidak dibalik menjadi Hamzah, Farid.
On the participant list, Farid bin Hamzah is not inverted to Hamzah, Farid.
EYD V normally writes internal elements such as bin and binti in lowercase while capitalizing the personal names around them. Preserve an individual's documented spelling when it differs; name correction is not an editor's casual decision.
Balinese name elements cannot be forced into middle-name boxes
Many Balinese naming systems use elements associated with gender, status, and birth order. Common birth-order sets include Wayan/Putu/Gede for a first child, Made/Kadek/Nengah for a second, Nyoman/Komang for a third, and Ketut for a fourth; cycles and local variation occur. I and Ni can mark male and female names in many contexts. A Kemdikbud-hosted linguistic study documents these patterns and their variations.
Perkenalkan, nama saya I Made Surya Pradana; panggil saja Surya.
Let me introduce myself: my name is I Made Surya Pradana; just call me Surya.
Ni Nyoman Ayu Lestari menulis seluruh namanya pada formulir pendaftaran.
Ni Nyoman Ayu Lestari writes her full name on the registration form.
Unsur Made pada nama itu tidak boleh otomatis dipindahkan ke kolom nama tengah.
The element Made in that name must not automatically be moved to a middle-name field.
These are cultural patterns, not a decoding machine. Not every Balinese person uses every element; birth-order names can repeat in larger families; titles and personal choices vary. Ask how the person uses the name and preserve its order.
Titles and forms of address are not automatically name fields
Bapak/Ibu and their shortened forms Pak/Bu are respectful address terms. They can precede a name in conversation, but they normally do not become part of the nama lengkap field merely because a speaker says Pak Daniel or Bu Rahayu.
Perkenalkan, ini Ibu Rahayu, koordinator layanan baru.
Let me introduce Ms. Rahayu, the new service coordinator.
Selamat pagi, Pak Daniel. Apakah Bapak sudah menerima jadwalnya?
Good morning, Mr. Daniel. Have you received the schedule?
Pada kolom nama lengkap, tulis Daniel Simanjuntak, bukan Pak Daniel Simanjuntak.
In the full-name field, write Daniel Simanjuntak, not Pak Daniel Simanjuntak.
Capitalize Bapak, Ibu, Pak, Bu when they directly address or replace a particular person. Use lowercase for ordinary kin nouns: bapak saya, ibu mereka. Pak and Bu take no period.
Professional and academic titles have their own forms. dr. means medical doctor, while Dr. is an academic doctorate. Prof. and degree abbreviations such as S.E. use periods (formal).
Dokter Maya Prameswari akan memeriksa pasien berikutnya.
Dr. Maya Prameswari will examine the next patient.
Surat itu ditujukan kepada Dr. Maya Prameswari, bukan dr. Maya Prameswari.
The letter is addressed to Dr. Maya Prameswari, with an academic title rather than a medical title.
Under Permendagri 73/2022, educational, customary, and religious titles may be recorded on the family card and electronic identity card and may be abbreviated; educational and religious titles are not placed on civil-registration certificates. A general event form should therefore separate nama lengkap sesuai dokumen from gelar/sapaan, rather than guessing which title is part of which record.
Initials follow more than one pattern
EYD V's abbreviation rules distinguish an abbreviated name element from a pure set of initials. Each abbreviated element takes a period, as in A.H. Nasution. A pure initial form representing a whole name uses capitals without periods, such as STA for Sutan Takdir Alisjahbana.
Nama A.H. Nasution mempertahankan tanda titik pada unsur yang disingkat.
The name A.H. Nasution retains periods after the abbreviated elements.
Dalam indeks singkat, Sutan Takdir Alisjahbana dapat diwakili oleh inisial STA.

If the document gives the name in full, do not replace it with initials you invented.
An official record may preserve a person's established punctuation. The EYD distinction guides new edited prose; it does not authorize silent alteration of a legal identity string.
A registration form that does not invent surnames
A robust domestic or educational form begins with one undivided required field and makes analytical fields optional.

The rows are hypothetical models, not claims that every bearer of a similar string uses the same classification. A real form should also allow the registrant to correct imported data.

Introductions use the name the person offers
Nama saya... gives a name; Saya biasa dipanggil... offers a call name; Perkenalkan, ini... introduces someone else. Use the title that fits the relationship rather than treating it as a fixed grammatical prefix.
Nama saya Nadia binti Yusuf, tetapi di kantor saya biasa dipanggil Nadia.
My name is Nadia binti Yusuf, but at the office I usually go by Nadia.
Perkenalkan, ini Daniel Simanjuntak dari tim hukum.
Let me introduce Daniel Simanjuntak from the legal team.
Daniel: Panggil saya Daniel saja. Rina: Baik, Daniel.
Daniel: Just call me Daniel. Rina: All right, Daniel.
A person's offered form can override your initial guess about formality. Keep Pak/Bu where hierarchy or local convention calls for it; drop it when the person clearly establishes a different mutual norm.
Common Mistakes
1. Assuming the final word is always a surname
❌ Pratama pasti nama keluarga karena berada di posisi terakhir.
Incorrect—the final position alone does not establish a family name.
✅ Tanyakan kepada pemilik nama apakah Pratama merupakan nama keluarga.
Ask the name's owner whether Pratama is a family name.
2. Inventing a surname for a one-word name
❌ Formulir mengubah Rahayu menjadi Rahayu Santoso agar kolom nama belakang terisi.
Incorrect—the form invents an identity element to satisfy its own design.
✅ Formulir mencatat Rahayu sebagai nama lengkap sesuai dokumen.
The form records Rahayu as the full name shown on the document.
3. Treating bin or binti as a family name
❌ Nama Nadia binti Yusuf dibalik menjadi Yusuf, Nadia binti tanpa persetujuan.
Incorrect—the patronymic has been rearranged as though Yusuf were automatically a surname.
✅ Nama Nadia binti Yusuf dipertahankan dalam urutan resminya.
The name Nadia binti Yusuf is kept in its official order.
4. Putting an address title into the name field
❌ Nama lengkap: Bapak Daniel Simanjuntak.
Incorrect on an ordinary registration form—Bapak is an address title, not automatically part of the full name.
✅ Nama lengkap: Daniel Simanjuntak; sapaan: Bapak.
Full name: Daniel Simanjuntak; form of address: Bapak.
5. Lowercasing personal name elements indiscriminately
❌ Farid Bin hamzah mendaftar pagi ini.
Incorrect capitalization—personal names are capitalized, while internal bin is normally lowercase.
✅ Farid bin Hamzah mendaftar pagi ini.
Farid bin Hamzah registered this morning.
6. Adding dots to a pure initial form
❌ Inisial Sutan Takdir Alisjahbana ditulis S.T.A. dalam indeks EYD V.
Incorrect for the pure initial form—EYD V writes it without periods.
✅ Inisial Sutan Takdir Alisjahbana ditulis STA.
The initials of Sutan Takdir Alisjahbana are written STA.
